To better identify tumors HRD, we developed validated a transcriptomic signature for HRD PCa (HRD-P). METHODS: By using an established mutational tumor created 16-gene HRD-P six independent cohorts (primary PCa, n=8,224; metastatic castrate-resistant [mCRPC], n=328). Molecular clinical features were compared between HRD-P+ those single HR-gene that would qualified patients recent inhibitor trials mCRPC. RESULTS: more numerous than primary (201/491, 41% vs 32/491 6.5%; Figure 1) mCRPC (126/328, 38% 82/328, 25%) cases was predictive genomic instability suggestive HRD. shorter time recurrence following surgery overall survival men (Figure 2). In prospective trial treated olaparib (n=10), all three experienced prolonged (>330 days) PSA progression-free survival.
